For some trainings, Mind Synergy will require a medical referral. We require our students to be supervised by their physician for trainings related to pain, whether it is for birthing, dental, or medical pain, Dis-ease or changes such as breast enhancement. Pain is normally an indication of something being wrong. We do not wish for symptoms to be hidden from your Doctor. If your Doctor is aware of your condition and sees no problem with our training bringing you relief, then we will gladly teach you how to be distracted from the pain or discomfort. Additionally, we require a medical referral for breast enhancement. The referral forms we provide here are in .PDF format and may be downloaded from the links below. Alternatively, we can fax the form to your Doctor if you provide us with his fax number. The referral form must be signed by your Doctor and either
FAXed to us from the Doctor's FAX machine (showing the
Doctor's fax data imprinted from transmission to us, or it may be
hand-carried if the form is in a sealed envelope which also
has the Doctor's business identification imprinted on the
envelope. Plain envelopes will not be accepted.
